To maintain his word to Dr. Strauss, Sam plans to bring his next victim back to the house and kill him immediately afterward.
Alan dissuades Sam from killing Elias without pressuring him to harm both of his captives. Sam manages to hold off on killing Elias, but only just. Sam takes Strauss at his word and follows his advice to visit his ex-wife in an effort to change his mood.

Sam reaches out to a woman from his past; Dr. Strauss, Sam, and Elias face upcoming challenges.
In the next episode, the defanged patient tries to meet his ex-wife in order to prevent himself from killing Elias, marking a rare view into Sam’s past instead of Alan’s.

5. Episode 4 Recap
Sam has brought his most recent victim to his house, making Dr. Strauss his last remaining captive. At first, Sam rejects Strauss’s advice to distract himself from his impulse to kill, blaming his father for “fucking him up” and suggesting instead that he is incapable of doing so.
In contrast, when Alan begged him several times to stop killing and just go about his day at the office, Alan finally agreed. Sam makes it through the day without killing Elias, but the thought of doing so haunts him even at the office. Elias and Alan get close because they both have a phobia of being killed and are amused by Sam’s insanity.
Sam has returned and is now on a murderous spree to finally put an end to Elias. Despite Strauss’ best efforts to calm him down, Sam murders Elias and prepares to go on to the next victim before another family connection occurs to Strauss.
In order to shake off his current mood, Alan asks Sam to see his ex-wife. At first hesitant, Sam eventually follows orders and leaves. Alan soothes Elias’s fears by wondering aloud what may cause Sam to kill them.
